Click the link below for your "ARC" Redemption form and save it on your computer or simply print it out. The form is in a 'fillable' PDF format so you can use it any way is easiest for you.

Order services as you would normally, then make sure you get a job number from Amstar Express and enter it on your form along with the order date. Each job number equals one (1) point.*

When you earn enough points for your desired gift, simply submit the form by one of the following methods:
Email as an attachment to Process@Amstarexpress.com... or
Fax it to (818) 887-1256...or
Mail to our main office at 509 Marin St., Ste. 237, Thousand Oaks, CA 91360
